We expect some youth who are at a level of independence which makes likely the
chance of a rapid move to independent living. We expect some youth who could
function in a less restrictive foster setting, but supporting documentation
and identifying the appropriate home can not be completed prior to the
immediate need for placement. We expect some youth whose level of need has not
been determined due to emergency removal or risk indicators requiring a more
extensive evaluation prior to placement. We expect some youth to retain
employment in the community. We expect some youth to participate in public
school and/or college.
Youth who have serious emotional and behavior problems will be referred to
LHS's more restrictive settings as soon as it is clear this need exists. LHS
will provide crisis support for the Transition Program through on-call
therapists and supervision and provide for emergency care in its more
restrictive settings should a serious emotional or behavior problem erupt.
Youth who are able to function within a foster home will be referred to LHS's
network providers or referred to the placing agency for community placement
in foster care.
This setting is appropriate for children and youth you are not yet self
directed but are able to function in a group living situation within the
community without 24-hour direct supervision.
This program is limited to low risk children and youth. Children and youth
within this program will not need to be physically restrained. Children and
youth in this program will not need court supervision and will not commit
offenses that would lead to court supervision. Children and youth in this
program will not need supervision due to past sexual offending behavior.
Children and youth in this program will not need supervision due to active
Psychotic symptoms and related behaviors or active conduct disorder behavior.
Children and youth in this program will not need supervision due to an active
obsession with drugs or alcohol. Children and youth in this program will not
present active suicidal ideation and have not presented suicidal ideation
over a lengthy period of time prior to placement.
The program will provide one awake night staff 7 days per week.
Youth in this program should be transitioned from this program in less than
one year.
The staffing for this program will consist of a live-in married couple and one
night staff person. Child care workers will be hired to provide limited time
off for the live in couple.
